There are two steps to applying for this service.

  1. You must click on both the links and print the forms, then close that window and it will bring you back to this one where you can click on the other link. Print the form and close the window.
  2. Once you have completed the printing process you can then fill in the forms and fax or mail them to Christian Financial Ministries.

If you click on the link and a message comes up that asks if you want to open or save the file, click open, and then print the file. Then close the file and return to this window.
